ASTYM (Augmented Soft Tissue Mobilization)

What is ASTYM and How Does it Work?

The ASTYM System is a physical therapy approach that effectively treats chronic, nagging problems that result from scarring and fibrosis. Chronic soft tissue problems of tendons, ligaments and muscles are often of a degenerative nature rather than inflammatory. This is why anti-inflammatory medications or injections may temporarily reduce pain but not resolve the problems. The underlying fibrosis needs to be physically treated. The ASTYM system uses hand held plastic instruments (like those shown here) to scrape and mobilize scar tissue in order to stimulate the body's healing process. Manual and other treatments may also be used with exercise to guide the remodeling process.

What Problems Can Benefit from ASTYM?

Many conditions and parts of the body can effectively be treated including: carpal tunnel syndrome; tendonitis in the arms, shoulder or legs; hamstring strain and post surgical scarring. The concept on which the treatment is based is that excessive fibrosis is the underlying cause of problems.

What Does the Treatment Involve?

Your physical therapist will evaluate your condition, assessing strength, mobility, tissue quality and nerve function to determine the appropriate treatment. ASTYM treatments are given on nonconsecutive days, 1-2 times per week. A typical 30 minute session includes 15 minutes of treatment and 15 minutes of exercise and assessment. Less severe conditions can respond well in 2-4 sessions whereas difficult chronic cases may require 8-16 sessions.

What Does It Feel Like?

As the instruments are applied to the area you will likely feel roughness or bumpiness. You may feel some discomfort the first few sessions. Comments from patients vary from saying that the treatment feels good, feels ok, or hurts a little. If sharp pain occurs the treatment to that area is stopped. Patients also say that it may hurt and feel good at the same time or that it feels like the "right" hurt that will make the problem better.

What Can I Expect After the Treatment?

Some patients experience temporary tenderness or bruising of the tissue in the rough areas that is a part of the body's healing process. As the rough areas smooth out, your pain will subside and movement will improve.
